Provide training on an individual and group basis, offering classes in work behaviors, job skills, coping skills, management of disability, independent living skills, behavior management, social and recreational skills and/or other needed training areas. Assist with implementation of alternate programming as assigned.
Develop lesson plans for each class offered in program, and update program manual as necessary.
Develop and implement a complete individual training plan for each client in the program as required and to update the plan as needed. Promote client participation in discussion of plans, goals, etc.
Conduct formal staffings, which include the client and any ancillary agencies, on a regular basis in order to evaluate client progress and develop a training plan based on client needs and preferences.
Provide crisis counseling to clients as needed.
Assist all clients as needed in order to promote community based services and full community inclusion.
Actively engage clients, implementing instructional plans and activities designated by the Community Support Services Supervisor.
Assist in implementation and documentation of behavior programs.
Maintain data collection and other required documentation, i.e., behavior tracking sheets, behavior incident reports, data sheets, injury/witness reports, etc. Submit documentation in a timely manner to the Coordinator of Community Support Services.
Provide for client toileting needs and clean up as necessary.
Monitor client break/lunch periods.
Ensure secure storage and inventory of program supplies.
Participate in staffing and development of program plans whenever possible.
Other duties as assigned by the Community Support Supervisor, including driving agency vehicle on community based outings.
